All participants need a completed health form on file for camp. Health forms will be linked on the confirmation page once your registration is submitted as well as in a confirmation email sent to the guardian (or to the camper if they are over 18).

Healths forms are due Monday, November 16.

A $10 late fee will be applied for all late forms.
